Cancellation Policy
Unanticipated events happen occasionally in everyone’s life. In the desire to be effective and fair to all clients, the following policies are honored:
24-hour advance notice is required when canceling an appointment. This allows the opportunity for someone else to schedule an appointment. If you are unable to give 24 hours advance notice you will be charged the full amount of your appointment. This amount must be paid prior to your next scheduled appointment. If you purchased a series please remember to deduct one session.
No-shows
Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show.” They will be charged for their missed appointment.
Late Arrivals
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, it will then be determined if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the full session. Out of respect and consideration for your therapist and other people, please plan accordingly and be on time.
HIPAA We cannot share your personal information without your written permission. If you would like records released to your family, PT or doctor, you must sign a consent form in person.
Refunds are issued in the form of gift certificates. An exception will be made if you have moved 50 miles or greater from the office and/or have a serious illness. Refunds are calculated based on the regular price rather than the discounted package price. When you receive a package discount, it is for the full number of sessions. Previous sessions deducted from the prepaid package amount will be calculated as individual (regularly priced) sessions. For example, if you have used two out of three sessions of a $200.00/three session package, your refund would be $50.00, e.g., 2x$75=$150-200=-$50. The 24-hour cancellation policy applies to packages and prepaid sessions. Please call with any questions.
